Exosomes in Professional Skin Care:A Promising Regenerative Approach

Exosomes are currently considered one of the most exciting approaches in modern dermocosmetics. Originating from medical research, they are increasingly being incorporated into cosmetic products as well as treatments. In particular, their ability to modulate cellular communication processes makes exosomes a promising approach for supporting skin regeneration and for addressing various types of scars.

What are exosomes?
Exosomes, also known as small extracellular vesicles (SEVs), are tiny membrane-bound vesicles approximately 30 to 150 nanometers in size – significantly smaller than a skin pore. They are released by cells and act as biological messengers, helping to transfer signals and bioactive molecules between cells. In this way, they can influence how cells communicate and respond to their environment. Through this communication, exosomes can transfer biological information that may activate or modulate regeneration processes. They also contain bioactive molecules such as proteins, lipids, RNA, and growth factors, thereby supporting repair mechanisms, anti-inflammatory processes, and skin renewal. In addition, because of their very small size and their natural role in transporting bioactive signals, exosomes are also being discussed as potential carrier vehicles that may help deliver certain active substances more effectively within the skin.
Why are exosomes of interest in skin care?
Exosomes are of particular interest in dermatology and dermocosmetics because they may help support the skin’s natural recovery mechanisms. Research is exploring their possible role in modulating inflammation, supporting fibroblast activity, and encouraging a more organized formation of the extracellular matrix. These processes are especially relevant when skin regeneration and repair are desired. This is why exosomes are being discussed not only in anti-aging contexts, but also in relation to scarfocused applications, post-treatment skin support, hair regeneration, barrier support, and hyperpigmentation.
Sources of exosomes
Exosomes can originate from different biological sources, including human, animal, plant, and microbial sources. In Germany (or the EU), exosomes used in the medical field are primarily derived from the patient’s own material (autologous) or from plant sources. For cosmetic development, however, source, safety assessment, regulatory classification, and market-specific requirements are essential considerations.
From our perspective as a German manufacturer developing within the European cosmetics framework, plantderived exosome concepts are currently the most suitable and practical route for cosmetic applications. Moreover plant- and microalgae-derived exosome concepts are of particular interest because they may offer good compatibility with cosmetic formulations and can be associated with bioactive molecules relevant to skin care. Microalgae-based exosomes have also shown broad regenerative potential in initial studies. They can influence signaling pathways associated with key regeneration factors such as FGF-2 (fibroblast growth factor 2), which is essential for fibroblast vitality and the structural integrity of the skin. These mechanisms may help reduce oxidative stress and support the skin in its natural renewal processes.
Use in dermatology and professional cosmetics
In dermatology and professional cosmetics, exosomes are currently being explored as a potential regenerative approach, particularly in combination with treatments such as microneedling. They are also being investigated as part of post-treatment care concepts designed to support skin recovery after professional procedures.
At present, exosome-based concepts in dermatology and professional cosmetics are used primarily in topical applications and as part of treatmentassociated skin care concepts. Injectable use is a separate field and should be viewed very critically in light of current regulatory and safety considerations. Exosomes are also opening up new possibilities in several areas of dermatology and cosmetics. Cosmetic applications using exosomes may influence the skin on several levels:
Hair regeneration: Exosomes derived from the curcuma plant, for example, may help activate hair follicles, stimulate scalp cells, and support the hair root, thereby contributing to improved hair growth conditions.
Improved barrier function: Gojiderived exosomes may help upregulate the expression of protective genes such as loricrin, which is important for epidermal barrier function and may help reduce transepidermal moisture loss.
Hyperpigmentation: Initial scientific findings suggest that exosomes may influence melanogenesis at several levels. Early studies indicate that they can modulate inflammation-related signaling cascades, interact with melanocytes, and regulate processes involved in pigment production. Although research in this area is still at an early stage, these findings make exosomes a promising approach for pigment-related skin concerns.
In scar-focused applications, exosomes are of particular interest because scar formation is closely linked to complex repair processes in the dermis. After skin injury, repair mechanisms are activated, including the increased release of VEGF-A (vascular endothelial growth factor A), which promotesthe recruitment of repair cells, angiogenesis, new tissue formation, and wound closure.
Exosomes can accompany these natural processes by improving communication between repair cells and supporting fibroblast activity. As a result, the extracellular matrix, including collagen and elastin fibers, may be formed in a more organized way, which can positively influence scar development. However, further data is still needed in this area.
Outlook in medicine
Beyond aesthetics, exosomes are also being discussed at congresses such as the IMCAS World Congress as a promising field of medical research. Ongoing studies are exploring their potential in highly challenging indications, including rare blistering skin diseases such as epidermolysis bullosa, where researchers hope they may support wound healing and tissue repair. Exosomes are also being investigated in cardiology because of their possible regenerative effects, for example in helping heart muscle tissue recover after myocardial infarction. Although this field is still evolving, current research may provide important new insights into the future therapeutic relevance of exosomes.
Personal expert view
In my personal opinion as a dermatologist, the real potential of exosomes lies less in their use as a stand-alone trend ingredient and more in their role within a well-designed treatment concept. Especially in professional dermatology and cosmetics, they appear most meaningful when integrated into procedural protocols and followed by targeted post-treatment care. This is also the perspective from which we currently evaluate and develop exosome-based concepts.
